Printed form with manuscript inserts signed by G. Washington, J. Trumbull, and James Bradford.
On the same document is a citation to Nicholas Dill for the Badge of Merit, signed by John Lamb.
A holograph note written across the document refers to a Land Warrant, dated July 16, 1796.
On verso of the document in a printed paragraph, is stated that a definite Treaty of Peace is the condition for the validity of the discharge.
